Hospice Services

Life’s Doors Hospice has a dedicated staff of registered nurses, physicians, home health aides, social workers, chaplains and volunteers who provide the critical support needed by patients and their families - whether the patient is at home or in a nursing facility.




Theodore A. Walters, MD

CASE MANAGERS

are registered nurses who meet with patients, families and their physicians to design specific treatment plans based on individual needs. They also assess and monitor patient condition. Available 24 hours a day, they are trained in managing the symptoms that accompany serious illness. They also provide regular reports to the patient’s physician and ensure that home health, social and counseling services meet the needs of the patient and family.

HOME HEALTH AIDES
assist with day-to-day care such as bathing, dressing and hygiene issues. They offer personal care in a manner that is respectful of the patient’s dignity and independence.

MEDICAL SOCIAL SERVICES
help with financial, legal or social needs as well as offer emotional support for both patient and family members.

COUNSELING SERVICES
of social workers and bereavement specialists are available to deal with the emotionally charged issues of dying. They work with patients and their families, friends and loved ones.

THERAPIES
such as physical, occupational and speech are offered to help with mobility and self-sufficiency. Nutritional counseling is offered as well.

SPIRITUAL SERVICES
are available by a Life’s Doors Hospice chaplain at the patient’s request. Spiritual care is nondenominational and does not promote any specific religious belief.

VOLUNTEER SERVICES
provide respite for caregivers and assistance with household tasks and errands. Specially trained for hospice care, these volunteers offer loving hands and caring hearts.

BEREAVEMENT SUPPORT
is offered to surviving family members and loved ones from the time of admission to twelve months after the patient dies. Support groups are also available to assist with the grieving process.
