Hospice

Hospice brings dignity and comfort to terminally ill patients, helping them and their loved ones cope with, and find meaning in, the end of life. A special program that focuses on the physical, emotional, and spiritual needs of the patient and family, hospice allows patients to live out their lives in a familiar environment and prepare for death in their own way.

Life’s Doors approaches hospice with a long history of compassionate care and a full understanding of terminal illness. Our staff of Registered Nurses (RNs), Social Workers, Counselors, and Music Therapist are specially trained to provide your loved one and your family with the critical support you need during this difficult time.

We care for patients who live at home or at a nursing facility, and at an assisted living facility.

Our hospice teams consist of the following staff:
Case Managers are Registered Nurses (RNs) who meet with a patient and their family and physician to design a customized treatment plan. Available 24 hours a day, Case Managers assess and monitor a patient’s condition, provide regular reports to the physician, and ensure that treatment plans and services are meeting the needs of the patient and family. They are trained in managing the symptoms that accompany serious illness.

Hospice Aides assist with day-to-day care such as bathing, dressing, and hygiene issues. They offer personal care in a manner that is respectful of the patient’s dignity and independence.

Medical social service staff are master’s prepared. They help with financial, legal, or social needs and offer emotional support for both patient and family members.

A Bereavement Coordinator provides counseling services to help patients and their family and friends deal with the emotionally charged issues of dying.

Physical, Occupational, and Speech Therapists help with mobility and self-sufficiency.

A Reigistered Dietician is available to assist with nutritional counseling.

A Music Therapist, certified with a BA in music, is available at the patient’s request to help alleviate distress and improve comfort.

Volunteers provide respite for caregivers and assistance with household tasks and errands. Specially trained for hospice care, these volunteers offer loving hands and caring hearts. They also provide pet therapy upon request.

Spiritual Care is available through a National Board Certified Chaplain.

Bereavement Support
Life’s Doors offers bereavement support to surviving family members and loved ones from the time of admission to 13 months after the patient dies. Support groups are also available to assist with the grieving process.

Payor
Medicare, Medicaid, and most other insurance will cover the cost of hospice service. Life’s Doors will bill the insurance company on your behalf.
